Welcome! Our electronic forms streamline the check-in process. You'll receive a text or email before your appointment, asking you to complete them digitally. Completing these forms in advance is beneficial for us. Additionally, we may occasionally ask our established patients to update their information, although we aim to keep these requests minimal.

Billing & Insurance
NWPIM offers the convenience of direct billing to your insurance carrier. Please remember to bring your insurance card to each appointment and inform us promptly of any changes to your insurance information. However, it's important to note that while we bill your insurance, payment isn't guaranteed. You are responsible for any copays, deductibles, coinsurance, or services not covered by your insurance plan. If your insurance requires a copay for office visits, please make this payment at the time of your appointment.

Senate Bill 1529 is in full effect which means Oregon law now requires all patients to have a designated PCP in-network with their insurance plan. If you have Providence Health Plan and have not yet assigned Mindy as your PCP, please contact your insurance company July 1st to get switched! For all other insurance plans, you may select either your favorite NWPIM provider or the clinic as your Primary Care Provider, depending on your plan’s specifications. Don’t worry—you can still see all NWPIM providers regardless of which one you choose to be your PCP.

Medical Records
New patients
Please call 503-643-2100 to set up your new portal account in order to submit forms electronically.
Established patients
Please visit the portal to download the medical records form and submit by mail, portal, or fax (503.643.7300).
There may be fees for the release of records as permitted by state law (ORS 192.563) and HIPAA Privacy Rule 45 CFR 164.52(c)(4).
Transfer of records to a new provider will be sent complimentary. Subsequent new provider transfers will result in a $75 charge to the patient; payment is required for release of records. For personal copies of medical records, costs may range from $25-$300 (depending on the size). Payment is due prior to receipt.
